EVIDENCE INTO PRACTICE™

EVIDENCE into PRACTICE™ is a fully facilitated, paid for programme which aims to ensure that people with diabetes and those at increased cardio-metabolic risk receive optimal care through the effective implementation of national policy and guidelines, particularly NICE Type 2 Diabetes Guidelines (Clinical Guidance 87, May 2009).

This programme provides a supported, phased and practical approach to improving patient outcomes by:

  • Stratifying cardio metabolic risk in practice populations to enable health professionals to develop their services and match resource to need
  • Exploring evidence into practice, to help patients reduce their exposure to cardio metabolic risk and improve outcomes
  • Continuous professional development, integral to ongoing healthcare professional appraisals and the new revalidation system for GPs

Helps you learn more about the spectrum of cardio metabolic risk that exists across your practice population so you can develop your services and match resources to need

Helps you explore how to bring ‘evidence into practice’ to help your patients reduce their exposure to cardio-metabolic risk and improve outcomes

Since July 2009, more than 200 practices have taken part in the Evidence into Practice programme. Around 1.5 million people have been stratified according to their cardio-metabolic risk and hundreds of health professionals have started to work on improving their confidence in the management of diabetes and cardio metabolic risk.
